Endoscopic Brow Lift

Historically the brow lifts have been performed to elevate sagging eyebrows using a large incision either on the forehead or behind the hairline in the scalp. Objective is to release the muscles and the tissues so that the brows could be raised to a younger looking position.

Endoscopic brow lift was created to eliminate the need for large incisions and scars and decrease the complications and recovery. Only small incisions are needed to allow a small microscopic camera to be passed under the skin. Using micro instruments the tissues and the muscles are released and the brow is elevated. The recovery is much faster and there is less chance of damage to the sensory nerves.

Short Scar Facelift

By definition a short scar face lift uses an incision that is limited to the front of the ear and does not extend behind the ear.

It is used to improve the jowls and lower face laxity.

Based on the surgeons expertise the same small incision can be used to lift the muscles of the face by the extended SMAS face lift method. Since there are no skin elevations behind the ears or the neck the recovery is much faster with significantly less swelling and bruising.

One must keep in mind that this procedure is not suitable for those with significant neck laxity and the surgeon must be properly trained in performing facial plastic surgery.

Upper Eye Lid Lift

Upper eye lid lift, also known as blepharoplasty, is one of the most common procedures performed in a Facial Plastic Surgery Office. Most men and women start to see the signs of aging around their eyes in their early thirties. As time progresses the upper eye lid skin becomes more prominent and in some instances may interfere with their vision.

The procedure to correct this issue involves a surgical procedure that is usually performed under local anesthesia. For the upper eye lid lift, an incision is made in the crease of the upper eye lid to remove the excess skin and fat as needed. The objective is to bring back a more youthful and natural look. The incision is place in the fold and as time progresses it usually becomes very difficult to detect.

The recovery from eye lid surgery is very rapid and one can usually return to normal activity within a week.

Zerona Laser

Melt Fat With Zero Pain and Zero Down Time Using Zerona

Zerona Laser is a low level laser that penetrates through the skin and is specifically absorbed by the fat cells. The laser beam releases the liquefied fat molecules from the fat cells. Some of these liquefied fat molecules are excreted through the feces and urine and others are taken back up by the fat cells. Repeating the Zerona treatment causes more of the fat molecules to be excreted, helping to further reduce the size of the area treated.

On June 15th of 2009 Medicis® began it first shipment of Dysport™ to the physicians in the US. Dr. Moradi at Face Beautiful had been previewed to the FDA studies on Dysport™ during a special meeting in December of last year.

DYSPORT™ is an acetylcholine release inhibitor and a neuromuscular blocking agent, approved by the U.S. Food and Drug Administration (FDA) in late April 2009. The approval included two separate indications, the treatment of cervical dystonia in adults to reduce the severity of abnormal head position and neck pain, and the temporary improvement in the appearance of moderate to severe glabellar lines in adults younger than 65 years of age. Medicis® has rights to market DYSPORT™ in the United States for the aesthetic indication (glabellar line)

According to the American Society for Aesthetic Plastic Surgery, over 10 million cosmetic procedures were performed in the U.S. in 2008, 83% of which represented nonsurgical procedures. Injections of botulinum toxin type A have been the number one nonsurgical cosmetic procedure for the past five years, with over 2.4 million total procedures in 2008 alone.(1) The U.S. aesthetic market for botulinum toxin type A is estimated to be approximately $300 million to $400 million.(2)

DYSPORT™ Important Safety Information

The effects of DYSPORT™ and all botulinum toxin products may spread from the area of injection to produce symptoms consistent with botulinum toxin effects. These symptoms have been reported hours to weeks after injection. Swallowing and breathing difficulties can be life threatening, and there have been reports of death. The risk of symptoms is probably greatest in children treated for spasticity, but symptoms can also occur in adults, particularly in those patients who have underlying conditions that would predispose them to these symptoms.

The dosing units of DYSPORT™ are not the same as and are not interchangeable with those of other botulinum toxin products.
Recommended dose and frequency of administration should not be exceeded.

Reloxin® (Dysport) is now FDA approved

Reloxin® is a botulinum toxin type A protein very similar to Botox® that has been used in Europe and other countries around the world with similar applications as Botox®. It was just approved today in the United States and will likely hit the physician’s office next month. One of the most common applications will include reduction of lines around the eyes.

The difference between Reloxin® (Dysport) and Botox® will include the number of units injected and the price. Reloxin® (Dysport) will most likely be priced lower than Botox®.

How to choose a Plastic Surgeon

First you need to identify the issues that concern you. Sometimes is difficult to figure out what causes us to look older or appear tired.

Once you have an idea about the areas of your face or body that need rejuvenation you can start looking for a plastic surgeon who is an expert in that area and may be right for you.

Ask a friend:

It is always best to start your search through referrals from people who are familiar with the surgeons work. This is by far the most important aspect of the search.

Good cosmetic surgeons have a good reputation in the community. One must be careful that although a surgeon may be good at doing facelifts or Breast augmentation it does not mean he is automatically good at performing Rhinoplasty.

Go in for consultation:

This allows you to meet not only the surgeon but also the staff. The staff is a representation of the surgeon and will be taking care of you before and after the surgery. You can tell a lot about the surgeon by the way the staff takes treats you.

During the consultation you can ask for before and after photographs . Make sure all your questions are answered. It is best to get the details of the procedures and the risks involved during your first consultation. Most patients are so worried about the risks that they cannot fully concentrate on the surgeons explanation. It is best to ask all the hard questions first so that you can get past it to better things such as the results.

Talk to prior patients:

Surgeons with good outcomes usually have a number of patients that have volunteered to speak to other patients. These patients have already gone through the procedure and are a great source of information.

What about qualifications:

Qualifications include the training and experience that the surgeon has obtained to allow him to perform certain procedures. For example, to perform a face lift the surgeon must have the necessary training in facial plastic surgery, and have performed the procedure in his practice with great results. Board certification means that the surgeon has satisfied certain requirements that are standard in that discipline.

Claims of certification can be confusing. "Turf battles" sometime lead one group to claim superiority over others. In truth, however, oftentimes boards overlap. You may have a choice of good surgeons certified by more than one board, depending on the procedure you have in mind.

Here's a roster:

The American Board of Facial Plastic and Reconstructive Surgery focuses solely on facial plastic surgery. It requires that a surgeon have previously achieved basic certification in either otolaryngology/head-and-neck surgery or general plastic surgery. The surgeon must also successfully complete a two-day examination and submit a hundred patient cases for peer review. Surgeons certified by that board are "double-boarded." They have two certifications: a basic certification in either head-and-neck surgery or general plastic surgery and an advanced certification focused on facial plastic and reconstructive surgery.

The American Board of Plastic Surgery addresses general plastic and reconstructive surgery of the entire body, including areas outside the face, such as hands, abdomens and breasts.

Some board-certified ophthalmologists and dermatologists also perform some facial plastic surgery within the scope of their specialties.

Where will the surgery take place?

Many surgeons perform cosmetic surgery in their nearby ambulatory surgery centers designed specifically for facial plastic surgery. Some patients prefer this, because it can provide comfort, convenience and specialized focus on the patient. Look for a facility accredited by an independent national organization. Other surgeons operate only in the hospital. The cost and convenience of the facility can vary. You should make sure you are comfortable with the planned facility.

Make sure you understand the financial arrangements. Most cosmetic surgery is not covered by insurance. You will probably have to pay for at least part of the surgery in advance. Make sure you understand whether you will owe any amount if you decide to cancel the surgery. Watch out for high-pressure sales pitches. You want a surgeon more interested in your health than in your money.

In summary:

Your surgeon needs to be nice, well trained and caring with competent staff in a nice and certified facility. He must be excellent at the required procedure with superior results that are supported by his prior patients.

When evaluating my patients for face lifts, I allow them to express their concerns. I like to hear in their words, the issues that bother them and what they think needs to be done to correct those.

I obtain a complete medical history to make sure the patient can undergo a surgical procedure if needed. At that time while patients look into a mirror I examine their facial features and explain my findings. Once the patient and I have completed the initial evaluation, I explain a variety of options that may be available to rejuvenate the areas of concern. During the same session I review the details of the necessary procedures, and explain the risks associated.

Many patients are concerned about the jowls at the lower face and their neck laxity. Depending on the findings and the patient’s desires the options may include; Small Incision Facelift, Small Incision Mid Neck lift, Combination lower Facelift & Neck lift or a lateral neck lift. Majority of the patients require either a Small Incision Facelift or a Lower Face & Neck lift. I perform the deep plane and extended SMAS Facelift. This type of Facelift is the most technically difficult Facelift and in my opinion provides the best results.

Many cosmetic surgeons have never performed this procedure or may not have the technical comfort to offer this type of Facelift.Our staff and I spend significant time and effort to assure that the patients are well prepared prior to the procedure and that they have a pleasant recovery.

Neck Lift

Many women and men over the age of 45 become increasingly concerned about the laxity in the neck. The most definitive approach to correction is through surgery.

Neck lift is a term used to define rejuvenation and tightening of the neck. There are many different approaches to rejuvenate this area and the exact approach is dependent on the patient’s desires and the extent of neck laxity.

Midline Platysma Bands(small cords directly under the chin):

Usually presents in the early 40’s. It is due to loss of elasticity of skin and deeper tissues. The contraction of the neck muscles contributes to the severity which is more apparent due to the loss of fat between skin and muscle of the neck. Treatment may include a small incision under the chin and tightening the muscles. This procedure is usually done under local anasthesia and has minimal down time.

Excessive hanging skin of the neck:

More commonly starts in the late 40’s. It is caused by the laxity of skin and muscles of neck. The best procedure to improve this issue is through lateral and medial neck lift. That means besides making an incision below the chin there would also be incisions made behind the ears to elevate the excess skin and pull the skin behind the ears. The recovery may take about seven to ten days.

Although correction of hanging skin of the neck may be simple the procedure must be performed by a surgeon with extensive experience in this area.

Botox® Neck Lift

Botox® can be injected in the neck bands to relax the superficial neck muscles. These are the muscles that contribute to the hanging skin cords that can be seen in the neck. The physician performing the injections must have the proper training and have a thorough understanding of the neck anatomy.

The side effects are very rare but can be significant such as problems with swallowing. The use of Botox® in the neck is Off-Label and must be discussed in length with the physician experienced in these procedures.

Botox® is placed just under the skin in the neck muscle called Platysma. After a period of seven to ten days one notices tightening of the neck as a result of relaxation of the muscle.

This procedure will not work for individuals with significant skin laxity. In those instances only lower face and neck lift will be effective.

The aging of the baby boomers has ignited the innovation of newer and less invasive procedures so that one can continue to look young without having to take any time off of work.

Laser Hair Removal

Winter is the ideal time for hair removal. During the colder season we are less likely to have sun exposure and are better candidates for lasers. Also there is enough time until the beginning of the summer to complete the treatment sessions.

You can spend your summer hair free and feel comfortable in your new bikini or sleeveless tops.

Laser hair removal works by targeting the melanin (the dark pigmentation in hair and skin) at the base of the hair follicle. It destroys the cells that make new hair and as the treatments continue the hair becomes lighter and thinner. It eliminates stubble and ingrown hair.

There are many different lasers and light treatments for laser hair removal. Some are more effective and safer than others. One needs to do some research before starting their treatment.

Best Facelift

What is the best facelift?

Dr. Moradi believes the best facelift is one that makes the patient look more than 10 years younger and would last for their life time.

To accomplish this, the incisions must be placed at a location that is least visible. For the most natural lift the muscle layer under the skin must be released and elevated to accomplish a natural appearance. The goal is to make one look like their younger self and not someone else. Many advertised facelifts do not accomplish this and entice patients by advertising fast recovery, but at the cost of unsatisfactory results.

The surgeon performing the facelift must be properly trained and very skilled. There are many vital structures such as nerves and arteries in this area and the surgeon must be knowledgeable and meticulous to achieve a comprehensive lift with minimal risks to these structures.

Botox® rival Reloxin® could go on sale in U.S. next year

Botox® challenger Reloxin® could reach the U.S. marketplace next year.

Its corporate sponsors, Medicis® Pharmaceutical of Scottsdale, Ariz., and Ipsen filled their application for Food and Drug Administratin.

Medicis said it is hoping for FDA action on the application by early 2009.

Ipsen makes the drug, a dilute form of botulinum toxin similar to Botox, and sells it in about 20 countries under the name Dysport. If the two companies win FDA clearance, Medicis would market it in the United States under the name Reloxin.

Botox, made by Irvine-based Allergan®, is the No. 1 cosmetic medical treatment in the United States, with more than 3 million injections in 2006, according to the American Society for Aesthetic Plastic Surgery. Medicis estimated that the U.S. market for cosmetic botulinum toxin injections totals $300 million to $400 million.

Permanent eyeliner and brows

Are you aware that the 1st physician in the state of California to do permanent eyeliner is the senior member of the staff at Face Beautiful?

Dr. Antle (me) has been doing permanent eyeliner since 1983. I taught courses in it for several years to other physicians and nurses. Over the years, the eyeliner and brow business has gradually migrated out of the surgeon’s office into the domain of the beauty shop, and independent home practitioner.

Although many of these practitioners do a good job, we continue to see absolute horror stories. Be warned! Many non medical personnel, doing permanent eyeliner, have caused scarring, injured eyes, and produced some really terrible results. In addition, the procedure is often extremely painful since they are unable by law to properly anesthetize the eyes.

Micro Laser Peel uses a computer controlled superficial Erbium laser that treats only the very top layer of your skin. After the treatment the skin usually looks red and flaky which resolves in a few days. It does wonders for the fine lines, pores and superficial skin discolorations.

Liquid Lifting, a.k.a. Hollywood Liquid Lifting, is currently the buzz in the cosmetic press. Many movie stars, models, and other semi famous people are currently being plumped rather than stretched. Liquid lifting involves taking mainly hyaluronic acid fillers, either the Restylane® family or the Juvéderm® family, and filling out the cheeks and filling in the hollows of the face that have lost volume. This is an extremely effective way to make the face appear more youthful.

Although this appears to be the current rage, at Face Beautiful, Dr. Moradi and I have been doing this technique for years. We are San Diego County’s highest volume filler injector practice, along with the highest volume Botox® practice. The reason is our expertise.

Fillers injected close to bone in the face to restore volume, last much longer than when placed close to the surface in areas such as the lips. The longer acting hyaluronic acid fillers can last up to 2 years when placed close to the bone. The results are instantaneous, the procedure itself is virtually painless, and the cost is modest.

The usual volume necessary to restore youthfulness varies from 2 to as much as 5cc. Most younger women require a simple cheek lift, the older women may require more of the cheek, brow, and wrinkle filling effort to restore volume.
